Description
The Human Services Case Coordinator assists individuals and families by providing service coordination and advocacy while linking them to existing community and state resources. This position also assists individuals with budget administration and serves as a liaison with community members, state agencies and service providers.

JOB FUNCTIONS AND RESPONSIBILITIES:

•        Provides support and assistance to families per designated and applicable state of NH regulations and waivers.
•        Advocates and supports families and individuals in a manner which increases opportunities for participation in the community by responding to the changing needs of the family as a whole and balances the use of informal and formal resources.
•        Serves as a liaison with community and state agencies as well as service providers.
•        Works in cooperation with other team members regarding issues of quality supports and family/individual areas of concerns including utilization of services.
•        Supports and carries out the principles of self-determination and community participation within professional responsibilities.
•        Works with individuals and families to promote choice and control over the supports and services they receive.
•        Participates in educational/area agency transition meetings and planning and works in conjunction with Early Supports and Services staff as assigned.
•        Coordinates assessments and evaluations timely for all individuals as assigned. Secures all necessary referrals, documentation and relevant data to determine the individual’s unique needs including though not limited to the Support Intensity Scale (SIS) and Health Risk Screening tool (HRST).
•        Coordinates admission and discharge planning from hospitals in conjunction with nursing and other team members, as well as any other individual transitions and/or moves.
•        Facilitates legal process for individuals/families as necessary including Guardianship, Power of Attorney, Advance Directive planning and Representative payee.
•        Facilitates planning meetings for individuals including completing, compiling and processing Individual Service Agreements/Amendments/Action Plans and other related documents as requested or required.
•        Monitors the implementation and quality of services as outlined in the Service Agreement and applicable state regulations.
•        Maintains regular contact with families, individuals, and professionals and retains documentation of such contacts including though not limited to contact notes.
•        Participates in ON-CALL rotation as scheduled and shares caseload coverage of others as requested during vacations, sick time, emergencies or other times as assigned.
•        Performs other duties and responsibilities as assigned.

Skills
Bachelor’s degree in related field is highly preferred; High School diploma or GED required
2-3 year minimum case management experience
Minimum of 2 years human services experience a must; experience with developmental disabilities preferred.
Excellent verbal, interpersonal, and written communication skills required
Must be be flexible and able to effectively manage job fluidity
Intermediate proficiency in MSOffice (Word, Excel, PowerPoint) and ability and willingness to learn new programs a must.
Ability to multi-task and prioritize a must; ability to meet deadlines needed
Strong organizational skills required
Budgeting experience highly preferred.
Must be willing to travel regionally
